Historic House Plaque
Historic House Plaque
Order your historic house plaque for $75. Only properties located within the nationally registered “West Of The Boulevard Historic District” will be issued a West of Boulevard plaque*.
The date on the plaque will be the construction date recorded with the National Registry of Historic Places (for more information, visit the Department of Historic Resources). However, if you have the original deed that provides the year the house was deeded, please enter that date when you place your order. We will not be able to correct it after the plaque is cut, so please ensure the date is correct. If you do not enter a date, you agree to accept the date provided by the DHR.
*While the majority of our neighborhood includes West of the Boulevard Historic District, some blocks are actually in the Monument Avenue Historic District.
If you live on one of the blocks listed below, please check both registers linked above to find which Historic District applies to your home. If you are in the Monument Avenue Historic District and would like to order a house plaque, please contact historic.monument.avenue@gmail.com.

Multi-family buildings: the MDA does not issue house plaques for individual units in multi-family buildings. We can, however, issue it for the building as a whole with a purchase request from the owner of the building or the condo owners association.
